Update Your Information

Change of Name

In order for us to recognize a change to your name, please send the EQSF a letter requesting that we update your name.

In your letter, please be sure to tell us your new name and your previous name along with your Social Security or your EQSF Master ID number so that we are able to find you in our system.

You must also provide us with a copy of a government-issued piece of identification (driver's license, state ID, passport, social security card) or court document that establishes or confirms your new name.
