The EQSF's New Direct Deposit Program
How it Works
With our Direct Deposit program, each time the EQSF issues payments for categories that you have approved claims in, we will deposit your award electronically into your checking or savings account. You no longer have to worry about receiving and depositing multiple checks.

You do not need opt in to the program if you would still prefer to receive your award in the form of an actual paper check. However, issuing paper checks to each approved claimant in every claim category is a lengthy process. If you wish to have your distribution directly deposited to your desired account (and you have not done so already), you must request a form directly from the EQSF by calling our toll free number at (800) 397-7455, or by clicking here to request to receive a copy of the form in the mail.

How to Fill Out the Direct Deposit Form

Please carefully read all of the information contained on the Authorization Agreement Form completely. If you have any questions, please call our toll-free number at (800) 397-7455. If you are comfortable with the form, and wish to have your award deposited directly to your bank account, proceed with these instructions.
In order for us to ensure proper deposit of your award, EVERY FIELD IS REQUIRED to be filled out on the form. If even one piece of information is blank, missing or incomplete, we will not be able to process your request for direct deposit.
1 - Checking Account/Savings Account
Place an "x" in the box to the left of the type of account (either a checking account or a savings account) you wish to have your award deposited to. Please only select one box.
2 - Financial Institution
Enter the exact name of your bank, credit union or other financial institution.

3 - Branch
Enter the name of the branch where you typically do your banking. Often, this will be the name of the city in which the branch is located. Please call or write to your financial institution if you are unsure.
4 - City/State/Zip
Enter the city, state and zip code of your financial institution. Do not enter your home or business address.
5 - Routing Number
Enter the routing number listed your check or deposit slip. This will be a nine-digit number, and is used to identify your bank. If you are unsure of the correct routing number, many financial institutions list it on their website, or you can call their customer service agents and ask.
6 - Account Number
Enter the account number listed on your check or deposit slip. This number will be located next to the routing number on most checks or deposit slips, and can be anywhere from 8 to 12 digits long. If you are unsure of your correct account number, you can call you financial institution's customer service agents and ask.
7 - Printed Name
Please print your name legibly. This is not your signature.
8 - Date of Birth
Please enter your date of birth (mm/dd/yyyy)
9 - Phone #
Please enter your ten-digit phone number (includes area code)
10 - Signature/Date
Please sign the document and enter the date you sign the document (mm/dd/yyyy)
11 - Notarization
It is very important that you have your form notarized by a licensed notary public, as allows us to verify that you are really the person who signed your form. Typically, you will be able to find a notary public at your bank who will notarize your form for a small charge or for no fee at all. You can also find a notary public through your local Yellow Pages.
Once you have found a notary, having your document notarized is a very quick and easy process. Typically, you only need to show the notary your driver's license, passport, or some other piece of photo-ID and they will notarize the document for you.
12 - Check/Deposit Slip
Whichever document you include, it should be taped (not stapled) to the form itself, in the middle, over the picture of a check. If you are unable to attach the check to the form (for example, you may have over-sized checks and it would cover some other information), please attach the check to a separate piece of paper. If you do send a check to us for processing, be sure to write the word "void" across the front, in large letters.
We use the document as a second source to verify your Account Number and Routing Number. By using two different sources, we can ensure the deposit of your funds to the proper account.
Please note that it is ok to include a deposit slip instead of a check, but it must be one of the personalized deposit slips taken from your check book. The document must be pre-printed with your Account Number and Routing Number, and may not be handwritten. The generic deposit slips (the ones kept at the bank) are not acceptable, as they do not have your Account Number pre-printed on them.
